Physical Turn in Day

Or the first day of try outs (please note if the physical is not correct your student will not be able to stay for try outs)
We will not be collecting physicals at Open House

Begin the first day of school - August 24, 3:45pm to 5:45pm
- 6th Graders cannot play football

Please make a copy of all the paperwork in case you need it for something else such as a camp, etc.

ALL students trying out for sports will need a new current physical, and concussion form turned in by the first day of tryouts.

Please don't wait until the first day of tryouts to turn in your physical - if it is not complete your student will not be able to stay or try out on the first day

Forms can be found on the NW Middle School website under Athletics.

Please turn in completed physicals and concussion forms by 8:30 am by the first day of tryouts to the Athletic Director. Forms can be turned in before the first day of tryouts.

Completed forms can be turned in August 17th from 1:00pm - 6:00pm in the front commons

We will NOT be collecting physicals at open house

7th and 8th grade students wishing to participate must have a current physical (current for 395 days), a current concussion form filled out in August to turn in on the first day of tryouts and meet eligibility standards (found on NWMS Athletic website). Forms needed can be found on the NWMS Athletic website or picked up from your PE Teacher.

There will be a mandatory parent meeting following tryouts to review county, school and team rules.

All athletes chosen for the team will need to pay a $45 Athletic fee. Managers do not pay the fee.

Parking for home NWMS Football games - turn right just past the Tennis courts - follow the road down the hill to the football field. Parking at the NWMS is very limited.
