NW Middle School Boys Basketball

ALL students trying out for basketball will need a new current physical and concussion form available on the NWMS website under athletics turned in by 8:30 am by the first day of tryouts. Forms can be turned in by Nov. 14. Forms can be printed from the NW Middle website under Athletics.

 Please turn in completed physical and concussion forms to the Athletic Director by 8:30am by the first day of tryouts. Forms can be turned in early.

In order to try out you must have a current physical and concussion form, and meet all eligibility requirements (found on NWMS Athletic website). Physicals are current for 395 days. Concussion forms are current for the school year so a new form must be turned in each school year. Forms needed can be found on the NWMS website under athletics or picked up from your PE Teacher.

 Mandatory Parent meeting TBD after team is chosen.

Athletes who make the team will need to pay a $45 Athletic fee online using the K!2 payment system found on the NW Middle website or cash or check at NW or send a waiver to GCS Director of Athletics before the first game to participate. This is a once a year cost. If an athlete paid the fee earlier this school year, he will not need to pay again. Managers do not pay the fee.
